Here is a copy of the "Project Management Process Groups and Knowledge Area Mapping" grid found on page 43 of the PMBOK (4^th editon). Although projects flow between the groups, the grid compartmentalizes actions into initiating, planning, executing, monitoring and controlling, and closing projects.
